The privacy, confidence, and trust of individuals who visit Affinity Health Center’s web site are important to us. No personal information is collected at this site unless provided voluntarily by an individual while participating in an activity that asks for the information. Examples of this would be: contacting us through our online contact form, processing an online donation, or signing up for our email newsletter. If you choose to provide us with personal information, as in an e-mail message, Affinity Health Center will use this information to respond to your request. There are times when your e-mail is forwarded within Affinity Health Center to employees who are better able to assist you. Please do not put confidential information in your e-mail communications to Affinity Health Center. You have the right to request that your personal information be removed from the website, affinityhealthcenter.org, by emailing your request to info@affinityhealthcenter.org. Affinity Health Center will not disclose personal information collected from users to private third parties unless the law requires such disclosure or it is a needed to process the transaction you initiated. Third parties we use include Mailchimp to process email newsletter lists and PayPal to process online donations.
Registered patients may access Affinity Health Center's Patient Portal through a link on this site. The Patient Portal is a separate website with it's own privacy policy.
