At Affinity Health Center, we make sure we measure up to meet the healthcare needs of our community. Sharing monthly summaries of patient demographics and visits with our staff, and with you, help us measure how well we're meeting these needs.

2017 Highlights for Quality of Care at Affinity Health Center


  1. adult measuring waist circumference.In 2017, Affinity Health Center set a goal to increase the percentage of adult patients who were given a follow-up plan to address their abnormal Body Mass Index (overweight or underweight). Affinity’s goal was 50% of overweight/underweight patients would have a follow-up plan. The National Community Health Center average for this is 59%. The South Carolina Community Health Center average for this is 57%. By December of 2017, Affinity Health Center surpassed our goal, the state average and the national average with 66% of overweight/underweight patients having a documented follow-up plan in place.

  2. In 2017, Affinity Health Center set a goal to increase the percentage of pediatric patients who received nutrition and physical activity counseling during their visits. Affinity’s goal was 54% of children seen at Affinity would receive counseling on nutrition and physical activity during the year. The National Community Health Center average for this is 58%. The South Carolina Community Health Center average for this is 51%. By December of 2017, Affinity Health Center surpassed our goal and the state average with 57% of children receiving nutrition and physical activity counseling.

  3. In 2017, Affinity Health Center set a goal to increase the percentage of adult patients who were screened for tobacco use and received tobacco cessation counseling or other intervention when needed. The National Community Health Center average for this is 83%. The South Carolina Community Health Center average for this is 81%. By December of 2017, Affinity Health Center surpassed our goal, the state average and the national average with 97% of patients being screened for tobacco use and provided a tobacco cessation intervention if needed.

  4. In 2017, Affinity Health Center set a goal to increase the percentage of patients diagnosed with HIV at the Health Center who were seen for follow-up within 90 days of being diagnosed. The National Community Health Center average for this is 75%. The South Carolina Community Health Center average for this is 82%. By December of 2017, Affinity Health Center surpassed our goal, the state average and the national average with 100% of persons newly diagnosed with HIV entering care within 90 days.

  5. In 2017, Affinity Health Center set a goal to increase the percentage of patients screened for depression and the percentage of patients experiencing depression who were provided a same day plan to address their depression. The National Community Health Center average for this is 51%. The South Carolina Community Health Center average for this is 35%. By December of 2017, Affinity Health Center surpassed our goal, the state average and the national average with 73% of patients being screened for depression and if depressed, provided a same day plan for follow-up.

Affinity Health Center has continued to struggle to assist patients with receiving colorectal cancer screenings. We are partnering with the S.C. Colon Cancer Prevention Network to improve access to these important screenings that can save a life!
