Our Story
Affinity Health Center is a Federally Qualified Community Health Center and proud to be part of America's network of community health centers. We are a medical home providing quality comprehensive medical care and supportive services for our patients in and around York County, SC. Our team includes Physicians, Nurse Practitioners, Dental Professionals, Mental Health Counselors, Nurses, Medical Office Assistants, Social Workers, Patient Access Specialists, Medical Billing Personnel, IT, Quality & Data Specialists, Educators, and Administrators,
Affinity Health Center receives HHS funding and has Federal PHS deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
2000: Nonprofit Status
In 2000, Catawba Care Coalition received status as a 501(c)(3) not-for-profit organization, expanded to include more case managers and an on-site medical clinic, and moved to its own facility in Rock Hill, continuing to expand and merge with other agencies to alleviate duplication of services and maximize limited resources.
2003: Continued Growth
In 2003, Catawba Care Coalition merged with the Catawba AIDS Prevention Network for consolidated prevention, outreach and education services, and also added HIV testing/prevention services.
2013-15: Extended Services
In 2013, Catawba Care added on-site dental work, and the next year, we began expanding our healthcare services to a broader community.
2017: Affinity Breaks Ground
In 2017, Affinity announced plans for a new 30,000 sq. ft. facility to expand healthcare access to the community.
2020: 20th Birthday Party
Affinity Health Center celebrates it’s 20th birthday with a party attended by old and new board, staff, partners and patients.
1994: Catawba Care is founded
The Catawba Care Coalition was formed in 1994 to serve people living with HIV in York, Chester, and Lancaster Counties. Originally, the agency was housed at the York County Health Department, and operated under the United Way.
2001: HIV Specialty Care
In 2001, Catawba Care Coalition merged with the York County AIDS Task Force, and began medical care services, including primary care and HIV specialty care.
2010: New Building
In 2010, Catawba Care Coalition moved to a new location under the simplified name Catawba Care.
2015: Affinity Health Center
In 2015, Catawba Care was rebranded as Affinity Health Center, and emerged as a full-service family practice, expanding to offer a wide range of affordable, high-quality healthcare services.
2018: A New Vision In Community Health
Affinity Health Center opens a new vision in community health with a new 30,000 sq. ft . building to better serve our community.

The privacy, confidence, and trust of individuals who visit Affinity Health Center’s web site are important to us. No personal information is collected at this site unless provided voluntarily by an individual while participating in an activity that asks for the information. Examples of this would be: contacting us through our online contact form, processing an online donation, or signing up for our email newsletter.
