Affinity Health Center operates with a staff of 59 full-time and 4 part-time employees. Our team of providers includes an Internal Medicine/Infectious Disease Physician, two Family Practice Physicians, three Family Nurse Practitioners, a Certified Nurse Midwife, Dentist, Mental Health Counselor, contracted consulting Psychiatrist, and Registered Dietician. We also have clinical support staff, case managers, and community health educators to provide comprehensive health services for you and your family. Family Nurse Practitioner - Full or Part-time

Building / Grounds Technician


Our growing company is currently in need of a responsible individual to fill an entry-level position as the Building and Grounds Technician. This position will be responsible for inspecting, maintaining and repairing mechanical, electrical, plumbing, and HVAC systems to ensure optimal operating results. This position is also responsible for making sure our building looks clean, presentable, and inviting at all times – especially in between routine cleaning and maintenance activities. The Building and Grounds Technician ensures the building is safe for all occupants, in compliance with OSHA and in the event of unexpected incidents or emergency situations. The successful candidate will perform routine janitorial responsibilities, manage staff requests for building assistance, facilitate emergency response and security protocols, and manage furniture and supply moves while remaining in compliance with local, state and federal regulations.


Requirements

  • High School Diploma required; certificate or degree in related field preferred.

  • Minimum 3 years experience in general facility and equipment maintenance

  • Experience with building systems and through knowledge of facility equipment

  • Knowledge of OSHA and crisis management preferred

Candidate must also be willing to travel locally to satellite office(s).

Patient Access Specialist - Full Time


Affinity Health Center seeks a full time Patient Access Specialist at our satellite sites to provide excellent customer service to patients; answer shared calls on agency call center; schedule patients for appointments; verify insurance and collect copays; complete medication assistance forms; collaborate with coworkers across the organization to provide patient centered care.


High school diploma or equivalent required; any combination of training and experience which would provide the required knowledge and abilities to perform the job duties; flexibility with hours, some evenings required; ability to travel to satellite sites which includes Clover, York and Ft. Mill; ability to multi-task in a fast paced environment; bilingual in Spanish preferred.

Mental Health Counselor - Full-Time


The Mental Health Counselor:


  • Manages delivery of safe and effective mental health services and patient care.

  • Provides mental health assessments, diagnosis and counseling, and collaborates with medical providers for coordinated care.

  • Establishes patient care goals to promote independence through patient-centered principles.

Education and Experience

  • Must be proficient with Electronic Medical Record

  • Bilingual preferred
