At Affinity Health Center, we want to make your first visit as smooth as possible. We ask for information to help you get the care you need at a price you can afford. You'll find it helpful to prepare some information before you come in for your appointment. It's a good idea to write down any questions you want to ask about your medical care so you don't forget. We're ready for most situations but let us know if you need special assistance like an interpreter. If you have questions or need to reschedule your appointment call us at 803-909-6363.
What to Expect
Your first visit is usually a consultation where you and your provider get to know each other. Follow-up appointments will be made for medical issues, annual physicals, or lab work. Specialty referrals will be made if needed.
What to Bring
Please download and fill out your new patient forms and bring them with you to your appointment. If you have no income, fill out the Income Statement form. You can fill these out on a computer and print them or print the blank forms to fill out by hand. Please review our Notice of Privacy Practices that explains how we keep your protected health information safe and confidential.

Behavioral Health & Dental Services
Patients are required to establish primary care at Affinity Health Center prior to accessing Dental Services and/or Behavioral Health Services including Mental Health Counseling at Affinity.Help for HIV and/or Hepatitis C Treatment
Our Linkage Coordinator provides personal assistance to our clients living with HIV or seeking treatment for Hepatitis C. To make an appointment for these services call Connie Wilson at 803-909-9730. It's helpful to bring the following documentation: proof of HIV or Hepatitis C status, like a reactive HIV or HCV test result or most recent HIV viral load or HCV RNA lab report.
